HOW TO ORDER
For your convenience, we offer four ways to
order: online, phone, mail, or fax.
ONLINE ORDERS

PHONE ORDERS
You may call customer service at 904.994.4914 to
place your order. Our staff is available weekdays from 10 a.m.
to 5 p.m. E.S.T. and on weekends from 10 a.m. to
2 p.m. Please leave a message if we fail to
speak with you and we will promptly return your
phone call.
FAX ORDERS
To order by fax, click to print out our fax
order form. Fax the completed and signed form to
866-899-4886. Our fax line is available 24 hours
a day. Please include your daytime phone number
so we can contact you for your credit card
information.
PAYMENT METHODS
We accept Visa, Mastercard, and American Express
for online, phone, or fax orders. Mail orders
can be paid by credit card or check. Your check
must clear before we can deliver your order. Once a
check is received, it can take up to seven
business days to clear.
ORDER TRACKING
You will receive an email receipt with your order details shortly after submitting
payment. If you do not receive this email
confirmation, please contact us

ORDER MODIFICATIONS
If you need to modify an order after it’s been
placed, we can accommodate your request as long
as we have not produced or shipped your order.
All requests must be made by email (Email
Us).
Please be sure to include your order number with
your request. We will confirm the change by
phone or email.

We do offer customization. To discuss your

PRODUCT GUARANTEE
If you are dissatisfied with your purchase,
please contact us within 24 hours at

Time. We request that you retain all remaining
product and packaging materials so we can best
assist you. If your product arrives damaged, we
request that you contact us immediately after
receiving your package.
